2017-2018 Child Nutrition Public Release 

The USDA has made several changes to the 2016-2017 Free/Reduced lunch application. Please read the Letter to Household and instructions for filling out the new form. The application can be downloaded HERE


Section 3 – Page 37 items 9 & 10 of the Quapaw School District Board Policy

9. Parents are encouraged to pay in advance for student meals. If necessary, students may charge up to one (1) week’s meals at a time – up to $11.00 for elementary school students and $11.50 for middle school students. If payment is not made the following week, notification will be sent reminding parents that payment is due. If payment is not received within seven (7) days of notification the student will be served either a peanut butter and jelly sandwich or a cheese sandwich with milk until their account has been paid in full.

10. No charges may be made during the last two (2) weeks of school. No student will be permitted to enroll in a subsequent school until his/her account is paid in full.

To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at http://www.ascr.usda.gov/complaint_filing_cust.html, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
